Read before proceeding:

  • If you get to question 1 and it would not advance to question 2 please see below for additional instructions.
  • Please only use Internet Explorer.
  • If IE disappears after clicking Submit button then see below for additional information on how to turn the pop-up blockers off.
  • Make sure your resolution is set at least at 1024x768
  • In case of computer crashing or any other reasons that cause IE to close, simply sign back on with the same name and ID.  It should take you back to where you left off.

Additional Instructions:

  • To fix the question not advancing problem try the following for IE7:

    • Open Internet Explorer and navigate to Tools>Internet
      Options.  

    •  Under the General tab and Browsing history section click on "Delete..." then click on "Delete Files..." and click "Delete all" then click Yes. Back at the Browsing history section click on "Settings..." and make sure it's set on Automatically for Temporary Internet Files.
       

  • To turn off pop-up blocker in Internet Explorer click on Tools - Pop-up Blocker - Turn Off Pop-up Blocker. If you have other pop-up blocker you must turn it off as well (such as Yahoo, Google's pop-up Blocker, or any security suite that has pop-up blocking features.)
